The Problem

Clinical and operational data needed to move from a legacy EHR system to a new enterprise platform. The two systems didn't share a data model, and clinical staff depended on that data being complete and correct the moment the new system went live — there was no tolerance for silent data loss.

The Approach

Performed a deep-dive operational analysis of the legacy EHR to understand not just what data existed, but how clinical staff actually used it day to day, then built field-by-field crosswalks between the legacy and new schemas before any migration work began.

What I Built

  • Mapped detailed data crosswalks between legacy EHR fields and the new enterprise system's data model.
  • Coordinated a cross-functional team of 30 — clinical stakeholders, technical teams, and vendor contacts — through the migration.
  • Defined validation checkpoints to catch discrepancies before they reached production.
  • Sequenced the cutover to minimize disruption to active clinical workflows.
